Judi Ingram, Clinical Director and Chief Nurse and Dr DJ Brown, the medical director at Aspen Healthcare asked if we could devise a way to make sure that healthcare professionals remember the four key ways to improve patient safety. We started with a guide of how to harm patients, which is proving to be very memorable, and effective. As Judi says:
“Our safety data have improved dramatically. In one year, we’ve halved the number of Serious Incidents and Never Events, even though we’ve treated 15% more patients, our reporting rates are up by 25%, but those with any harm have reduced.”
